Payment PolicyNew PatientsAs a new patient to my office, you will need to anticipate payment in full for the services you have performed on the date of your visit. The only exceptions to this rule are patients who have flat co-pays (usually $5, $10, $15, or $20) with their insurance plans. The co-pay will be noted on your insurance card if your plan is set up like this. If you are not sure, call your insurance company, and they will be more than happy to tell you the specifics of your plan provisions. Established PatientsEstablished patients are also expected to pay their co-pay at the time of service. If your insurance does not have a flat co-pay, we request payment in full. Payment plans are available for established patients with large balances. In these instances, payment arrangements should be made prior to the service being provided. There will be a finance charge added monthly to accounts over 30 days old. Maternity PatientsPatients who are pregnant are charged a global fee for the Prenatal Care and Delivery of their baby. Depending on your insurance plan, there may be a requirement to pay a portion of Dr. Davis' fee prior to the birth of your baby. Please call our office to discuss the requirements for your plan. Finance ChargesThere will be a monthly finance charge of 1.5% of the outstanding balance for accounts over 30 days old. The minimum finance charge is $1.50. |
